I'm looking to book a trip for myself and my children. On the traveler info page, it states that info entered must match government-issued photo id. What type of identification would I need to get my kids (ages 15 and 12) checked-in for our flight?

None. Kids under 18 years old do not need identification. You can use whatever name they go by when booking the flight.

No identification is needed however is the full middle name needed for the boarding pass or is first and last name sufficient?

Under 18 years of age the name does not matter unless the child is also enrolled in TSA Precheck, then the name would need to exactly match how it was given to TSA. No PreCheck? Put whatever name you would like.
